Insurance: 

 

We accept most insurance carriers. Please have your insurance card or Medicaid card with you at the time of your visit. If we file your claim, you are responsible for only the required co-pay or deductible at the time of visit. If we are not a provider for your insurance, if you are not insured or we do not file with your insurance company, YOU WILL BE RESPONSIBLE FOR THE ENTIRE CHARGE AT THE TIME OF SERVICE. Please call our office in advance to verify we are a provider for your insurance carrier. 

Our contract with your insurance carriers requires us to collect the appropriate co-pay and/or deductible at the time of service. If, on occassion, you are unable to make this payment and we must bill you, please be aware that a $10 Billing Service Charge will be added to the amount you owe us. 

For those patients that are without insurance and are Self Pay, we can offer a Sliding Fee Schedule based on Income and Family Size. An application for the Sliding Fee, which must be completed for each visit, is available at the Front Desk. Please come prepared to verify your income with either your prior year's income tax return or your three most recent pay stubs. All information is kept strictly confidential. Please see the Office Manager if you have questions.
